1. Document Requirements	Details the required documents needed for the loan application process. 1-page, 8" x 11".
2. Credit Authorization	Authorizes Axis Mortgage and Finance to run a credit report of applicant(s). 1-page, 8" x 11".
3. Mortgage Fee Disclosure	Defines the role of the Mortgage Broker. 1-page, 8" x 11"
4. Mortgage Disclosure	Grants Axis Mortgage & Finance to process a loan on the applicant's behalf. 1-page, 8" x 11".
5. Loan Application	Form to be filled out by applicant(s) for loan processing. 5-pages, 8.5" x 11".
6. Good Faith Estimate	Form indicating estimated expenses of applicants. 1-page, 8" x 14"
7. Broker's Disclosure	Disclosure on the Role of Axis Mortgage& Finance. 1-page, 8" x 11"

Please note the page sizes and adjust printer settings accordingly.

Checklist, form #7 (doc. requirements according to category; i.e. salaried, self-employed, etc.), will indicate which documents will be needed to process your loan for approval. Initials are required at the bottom of pages 1 and 2 of the Loan application (calyx1003). Signatures are required on pages 3 and 4.
